In the state of Florida, if you are under 18 years of age, you need to have the consent form that is located at the bottom of this page printed and taken to get notarized.

It doesn't matter if coming with your parent/bringing your child with you, this form is still required and there is NO way around it. A parent must also be present, the parent who is on the consent form. We unfortunately experience a lot of issues with parents thinking they can somehow bypass this process and I want to make it all as clear as possible.

• You must print the form at the bottom of this page and visit a place that offers notary services, we do not offer notary services.

• This is not my rule, but a Florida Health Department regulation/requirement for all tattoo and piercing studios regarding minors in this state.

There's various places that offer notary services, your bank should offer notary services at no cost usually, UPS stores offer it as well for a small cost, usually around $10.
This needs to be done BEFORE your appointment, you can’t show up with just the empty page to the studio.

• If for any reason you show up without the form below already notarized, the no show fee gets charged, this is part of the studio policy that is agreed to when you book the appointment.

• If you’re a child/teen/under the age of 18 reading this, please pass this page to an adult.
Another issue is kids booking on behalf of their parents and them not paying attention to any of this. This mistake of theirs/lack of understanding is also not an excuse to show up without the necessary form notarized, there’s plenty of information on this site as well as the actual booking site reiterating this info under every single booking option specifically meant for minors. An adult should be reading all of this and booking the appointment so that they bring what’s required.

Please don’t read this and think this won't apply to you. I have too many parents who show up even after reading this thinking they don't need it, you'll only get turned away and charged the no show fee because it wastes the appointment spot. I genuinely understand that this is an extra frustrating step, but I don’t make the health department regulations, we just follow them.
